There is an option if you live further away but wish to register with this practice and this is called ‘an out of area registration’. This means you could register with us and attend here for appointments, however we are unable to carry out home visits. This service is by application only and is at the discretion of the GP.
Should you wish to register as an out of area patient, having read the information, please complete the registration form and inform the patient services team that you wish to register as an out of area patient. The request will be passed to the GP’s for consideration and you will be notified of the outcome. The GP’s decision is final.
